Purpose
• To improve, maintain and recover patients' health after discharge, to bring patient effective independent skill
• Properly use medical treatment resources and prevent readmission.
• Patient can use the familiar environment for support and care.
Object
• To co-operate government policy provide continues care
• Reduce family life impact and improve life quality of patient family
Service Group: The case who need home care individual.
Service criterion
1. For patients who are evaluated need home care by physician.
2. Transfer patients.
3. 1+2 and under regulation of NHI.
Application: Doctor to fill in order sheet for transfer to home care registered nurse, for evaluation & accept of the client. Require ID, Health Insurance card and stamp.
Service coverage
1. General health examination, patients' condition evaluation and the problem.
2. Teaching
3. For drain tube care
4. Wound care.
5. Intravenous drip injection
6. Ostomy care
7. Sampling to examine
8. Consultation with doctor ¡B dietician
9. Arrange follow up to out patient department and transfer process communication.
Office Time
Monday to Friday: AM8:00 to 12:00 , PM 1:30 to 5:30
Saturday: AM8:00 to 12:00
Other emergency cases may contact the operator or on-duty staff
Fee
1. Health insurance payment (Health Insurance card and stamp required)
2. Partial Payment: minimum premium10% follow Health Insurance rule, serious injury do not need partial payment
3. Special material needed extra fee.
4. Traffic expenses.
5. Final cost should base on the definite receipt.
